The Advanced Care Planning (ACP) project involves the work of caring professionals around the state with a commitment to providing information to the community on advance care planning. These materials have been developed to assist presenters in the project.

ACP Project Process details the steps involved in the project.
Presentation Evaluation Form to be completed by attendees of the presentations and collected by the presenter.
Presentation Report Form to be completed by the presenter and returned to the Center with the attendee evaluations.
Follow-up postcard template (MSWord format: side 1 and side 2), to be printed 2-sided onto Avery postcards (#8387 or compatible) for follow-up (see ACP Project Process for details).
