Communicating End-of-Life Wishes
The Colorado Center for Hospice & Palliative Care has numerous professionals around the state trained to deliver presentations on advance care planning and how to communicate end-of-life wishes.
The presentation is mostly geared toward the public but can be helpful for healthcare professionals too. It discusses how to think about the choices you might have to make at the end of your life, how to communicate what you want and don't want, and how to ensure your wishes will be followed. Specific information about the Colorado Living Will, Medical Durable Power of Attorney, and CPR directive is included.
